The National Clearinghouse for Long-Term Care Information, developed by the U.S. Department of Health and Human Services’ Administration on Aging, provides basic information and resources to help families start preparing for future long-term care needs, including planning steps, housing considerations and how to pay for the costs of care.

  • A consumer guide to assisted living residences
  • A directory of licensed adult home/assisted living residences across New York State, searchable by county (38 in Nassau County, 54 in Suffolk County)
  • Adult Home Quarterly Survey Reports, listing complaints and violations
  • A consumer guide to continuing care retirement communities (under Long term care)
  • Nursing home profiles across New York State and estimated average rates
  • Complaints against nursing homes, including patient abuse and neglect.

In partnership with the New York State’s NY*Connects program, the Nassau and Suffolk county offices for the aging administer free, locally-based Connects programs that provide practical information, referrals, and assistance in accessing long-term care services and support for the frail elderly.

Funded by the counties and administered by local social service agencies, this program is designed to improve the quality of the day-to-day lives of elderly residents of nursing homes and adult homes. Trained and certified community volunteers investigate complaints and work closely with administrators, staff, residents and families to help resolve issues and to protect resident rights resolve problems.
