National Clearinghouse for Long-Term Care Information

The National Clearinghouse for Long-Term Care Information, developed by the U.S. Department of Health and Human Services’ Administration on Aging, provides basic information and resources to help families start preparing for future long-term care needs, including planning steps, housing considerations and how to pay for the costs of care.

New York State Department of Health

The agency offers extensive information about adult care facilities, including:

  • A consumer guide to assisted living residences
  • A directory of licensed adult home/assisted living residences across New York State, searchable by county (38 in Nassau County, 54 in Suffolk County)
  • Adult Home Quarterly Survey Reports, listing complaints and violations
  • A consumer guide to continuing care retirement communities (under Long term care)
  • Nursing home profiles across New York State and estimated average rates
  • Complaints against nursing homes, including patient abuse and neglect.

This website offers an extensive array of resources for caregivers, including nationwide directory, broken down by state, county and local community, of assisted living facilities, continuing care communities, nursing homes and other residential care options. Listings feature photos, consumer reviews, and amenities and cost information.

Empire State Association of Assisted Living


This trade association of assisted living providers in New York State offers tips and guidance for consumers on its website, in addition to a consumer directory of facilities, searchable by county.

Nassau NY Connects


Suffolk NY Connects


In partnership with the New York State’s NY*Connects program, the Nassau and Suffolk county offices for the aging administer free, locally-based Connects programs that provide practical information, referrals, and assistance in accessing long-term care services and support for the frail elderly.

Nassau County Office for the Aging


The agency’s website offers listings for adult homes, assisted living facilities and life care communities (Click Housing) and nursing homes (Click Medical, Nursing, Home Care & Long Term Care). The website has a link the Nassau NY Connects ( that has listings for adult homes and assisted living facilities, continuing care retirement communities, and nursing homes.

Funded by the counties and administered by local social service agencies, this program is designed to improve the quality of the day-to-day lives of elderly residents of nursing homes and adult homes. Trained and certified community volunteers investigate complaints and work closely with administrators, staff, residents and families to help resolve issues and to protect resident rights resolve problems.
