Next Step in Care

Next Step in Care is a program of the United Health Fund, an independent, nonprofit organization. The program is designed to provide guides to help New York family caregivers and health care providers work closely together to plan and implement safe and smooth transitions for chronically or seriously ill patients. The website offers a downloadable Family Caregiver’s Guide to Hospice and Palliative Care.
