Next Step in Care is a program of the United Health Fund, an independent, nonprofit organization. Nassau County Office for the Aging

Provides information about hospice care, including a listing of several Medicare-certified programs serving Nassau County residents. The website also has a link to NY Connects which lists more than 30 hospice programs in the region.

Through the VA’s Caregiver Support Program, local Caregiver Support Coordinators can arrange for palliative or hospice care services in a veteran’s home. If a veteran is deemed a terminal patient by a VA doctor, Veterans Affairs will pay for nursing home placement, if needed.
